STUDENT ENROLLMENT AGREEMENT
This agreement governs the enrollment for the online DISTANCE LEARNING PROGRAMS. I am fully aware that this document, and such attachments as may be listed below constitute my entire agreement with the school. Verbal agreements and/or other representations are not binding to the program. Included in the cost is an Application fee of $25.00 and a Processing/Handling fee of $50.00. The Distance Learning Home Study Program is divided into three sections. The home learner can complete each unit as quickly as they like or take up to 6 months to complete each unit. I am required to furnish my own grooming tools and text books, which are available on the general market or may be purchased while making payment at checkout. Purchasing tools and textbooks from the program is optional but you must use the textbooks specified. The list of required tools is shown in the catalog. At such time as I take possession of text books and tool kit, the said text books and tool kit becomes my property and responsibility. In addition, I may be dismissed should I not conform to the School’s policies or should I not maintain satisfactory progress.
Upon successful completion of this program, I will receive an acknowledgment of completion. Any student not current with their payment may be denied services including training, graduation, placement, etc, until the student’s account is current. Although the program cannot guarantee employment, I am aware that placement assistance is available to me upon my successful completion of course. A student may voluntarily withdraw at any time from any of the three Paragon Distance Learning Career Programs including the Stylist Assistant, Basic Groomer or the Pet Stylist Program. In such cases, a letter or e-mail to the director from the student is required. A student may be dismissed for any infraction of the school rules or policies which are listed in the school catalog. The termination date for refund computation purposes are as follows:
TERMINATION OR WITHDRAWAL AFTER COMMENCEMENT OF CLASSES – A student may voluntarily withdraw at any time from any of the three Paragon Distance Learning Career Programs including, the Stylist Assistant, Basic Groomer or the Pet Stylist Program. In such cases, a letter or e-mail to the Director from the student is required. A student may be dismissed for any infraction of the school rules or policies which are listed in the school catalog. The termination date for refund computation purposes is the date of the certified letter or e-mail received by the school from the student or the date the student was terminated due to an infraction of the school rues. Refunds of tuition are as follows:
The school may retain the $25.00 Application Fee and $175.00 Tuition Processing/Termination fee plus:
A. 25% of the total tuition if termination or withdrawal occurs between 4-7 days of the signed enrollment agreement.
B. 50% of the total tuition if termination or withdrawal occurs between 8-14 days of the signed enrollment agreements for each program.
C. 100% of the total tuition if withdrawal or termination occurs after 15 days of the signed enrollment agreements for each program.
All refunds due to the student shall be refunded within 30 calendar days from the date of termination of enrollment of student by the school or receipt by the school of notification from the student of withdrawal from the student.
The Distance Learning Program Handbook outlines complete rules of conduct and satisfactory progress. You can only begin your online courses once you acknowledge and agree to the conduct policies therein.
IMPORTANT ACKNOWLEDGEMENT – Paragon is a proprietary school licensed in the State of Michigan. The Acknowledgement of Completion of Paragon’s Distance Learning Program is not a license or certificate granted on behalf of any state or federal government and Paragon’s Distance Learning Program is not accredited by any branch of any state or federal government, organization, or institution.
Your first lesson will require a response to both the above acknowledgment and complete rules of conduct and satisfactory progress found in the Distance Learning Program Handbook.
